Welcome to Collins Elementary PTA!
We are parent volunteers collaborating with Collins staff to enhance and enrich the experience of our students. We support music, art, PE, and library programs, as well as organize assemblies and social events throughout the school year. Be a part of the PTA!
Your membership, donations, and volunteer support is important in funding various programs and activities that PTA sponsors for Collins students.

Yogurtland Fundraiser

Come, Support us at Yogurtland on
Tuesday, August 19!
We are very excited to bring our first fundraiser at Yogurtland (19700 Stevens Creek Blvd, Cupertino) on Tuesday, August 19 from 12:00pm - 8:00pm.
20% of the total sales will be donated to our PTA. Just bring the flyer and say the magic words "Collins Elementary PTA Fundraiser" to the cashier.
Watch out for the Yogurtland information flyer with your welcome packet or download an extra copy for yourself or a friend.

Please email Volunteers@CollinsPTA.org once you have complete your fingerprinting and submitted your TB results to the school secretary so we can get you added to our approved volunteers list.
Live Scan FAQs
Live Scan FAQs
The LiveScan is valid for as long as your child is in the Cupertino Union School District (CUSD).
Live Scan’s submitted to other agencies do not apply for CUSD.
If you had already done this previously for CUSD, you do not need to do it again.
You will not be receiving a receipt or copy of the Live Scan.
The school will be automatically be informed once the Live Scan has been completed.
Results take 1-2 weeks to reach the school.
The school will inform you if there is a problem.

Live Scan Fingerprinting
Who: To all Volunteers (on-campus or field trips)
When: Thursday, August 21, 2025, 4:00pm-6:00pm
Where: Collins school
(exactly location TBA)
Cost: $32 credit card, Apple pay, or Google Pay (cash and check not accepted)
What to bring: Government-issued identification document such as:
Driver's License (Physical ID required)
State ID (photos or copies of IDs are NOT acceptable)
US Passport/Passport Card
Foreign Passport with Valid Visa
You may fill out the Request for Live Scan Service form in advance. This form has pre-printed CUSD information and can be found HERE.
In addition to a fingerprint clearance, a TB clearance is also required. The TB Risk Assessment form can be found on the same CUSD volunteer webpage.
To volunteer or work in CUSD as a non-CUSD employee, you must:
Complete the CUSD volunteer agreement form. This form now includes the COVID training.
Complete fingerprint clearance for CUSD
Submit TB clearance to Ms. Arathi Sreeram
Note: If you have been fingerprinted by or for CUSD in the past, please notify Ms. Arathi Sreeram. You will not need to be fingerprinted again. However, TB clearance may still be required - each volunteer needs to have a risk assessment every four (4) years to determine whether a TB test is required.

*On the back of drawing, with light pencil, write your full name, grade, room number, and teacher.

Our biggest fund raising is from our direct donations campaign. From the generosity of families like yours, we are able to fund so many activities and programs for our students!
Collins Elementary PTA suggests a donation of $350* per student. You may donate above the suggested amount, and any amount given greatly helps our students and school.
We have a fundraising goal of $120,000 for the year. Please help us reach our fundraising goal!
All donations are tax deductible. Non-profit Tax ID 77-0034798 - 100% of your donation to Collins PTA stays at Collins to benefit your child.
