Authorization for Use and/or Disclosure of Information
Please complete the Authorization for Use form for us to include your information in our various PTA programs. This is required for student information to be shared for Walkathon or Running Club participation.

Parent's Approval, Student, Family, and Participant Waiver
Student waivers are required to participate in PTA activities (ie, Walkathon).
Sent completed PDF of PTA Reimbursement Form with PDFs / scanned receipts to Finance@CollinsPTA.org
All receipts are due by April 30.
Receipts must be dated on/after July 1 to be reimbursed for this school year.
