Do you like to make friends? How about help other people, even if it is for 15 minutes? Are you interested in engaging with your child and their fellow schoolmates more? We hope so! By getting involved with PTA and volunteering you become a solution for the school!
Without parent volunteers, programs and events could be cancelled. Please fill out the form below to get notified of volunteer opportunities or to tell us how / where you would like to help. We're flexible - help in any capacity that suits your schedule is welcome. You can do it from home at your leisure or visit during school hours, volunteering is flexible and rewarding!

Volunteering at Collins (and for Class Field Trips)
We're happy to bring many old and new Collins programs and fundraisers. With all these new events (and field trips!), we are always in need of supportive parent volunteers. Without your help, we couldn't bring these fun programs to our students and families.
Many parents have asked how they can get more involved and become a parent volunteer. Here are the basics to getting cleared to volunteer for our school events:

Expand to read the volunteer requirements.
Select Collins to complete our school's Volunteer Information Agreement. You will need to complete this form every year.
Download, complete, and bring the Request for Live Scan Service form with you to your fingerprinting appointment. (Don't forget to make an appointment; CUSD has an agreement with The UPS Store #4636 for discounted services.) Mrs. Taormina or Ms. Sreeram can check for your clearance. Once you have been cleared once for CUSD, you do not need to repeat the fingerprinting.
Download and complete the TB Risk Assessment form with your doctor and return to Ms. Arathi Sreeram (sreeram_arathi@cusdk8.org) or complete your TB test and submit a copy to Ms. Sreeram. You will need to resubmit your TB information every 4 years.
Get yourself cleared to volunteer sooner than later! The process can take up to a month depending on various factors.
Live Scan FAQs
The LiveScan is valid for as long as your child is in the Cupertino Union School District (CUSD).
Live Scan’s submitted to other agencies do not apply for CUSD.
If you had already done this previously for CUSD, you do not need to do it again.
You will not be receiving a receipt or copy of the Live Scan.
The school will be automatically be informed once the Live Scan has been completed.
Results take 1-2 weeks to reach the school.
The school will inform you if there is a problem.
Live Scan Fingerprinting
Who: To all Volunteers (on-campus or field trips)
When: Thursday, September 12, 2024, 2:15pm-4:15pm
Where: Collins school - Tech Lab
Enter through the main gates, first door on the right
Cost: $32 credit card or Apple pay (cash and check not accepted)
What to bring: Government-issued identification document such as:
Driver's License (Physical ID required)
State ID (photos or copies of IDs are NOT acceptable)
US Passport/Passport Card
Foreign Passport with Valid Visa
You may fill out the Request for Live Scan Service form in advance. This form has pre-printed CUSD information and can be found HERE.
In addition to a fingerprint clearance, a TB clearance is also required. The TB Risk Assessment form can be found on the same CUSD volunteer webpage.
To volunteer or work in CUSD as a non-CUSD employee, you must:
Complete the CUSD volunteer agreement form. This form now includes the COVID training.
Complete fingerprint clearance for CUSD
Submit TB clearance to Ms. Arathi Sreeram
Note: If you have been fingerprinted by or for CUSD in the past, please notify Ms. Arathi Sreeram. You will not need to be fingerprinted again. However, TB clearance may still be required - each volunteer needs to have a risk assessment every four (4) years to determine whether a TB test is required.
